Fields on the Family Details Form

Refer to the following tables for descriptions of the fields on the Family Details form.  An asterisk (*) indicates required information.

Some of the dropdown lists contain many options, which are presented in alphabetical order. To quickly find the option you want, type the first few letters of the option into the field. As you type, options matching the letters appear in the list. When you see the option you want, select it and press Enter.

To make changes to your family's information, you must put the Family Details form in edit mode by clicking edit details.png

Family Information 

Field

Description

Family Photo

If the family added a photo to their record, the photo appears next to their name. If a placeholder photo is shown, it means that the family did not add a photo to their record.

Church of Registration Shows the name of the church in which the family is registered.
Modified On Shows the date and time when the record was last modified.
Last Name* Last name for the family record.
First Name(s) * First names of the primary adults in the household. If your family consists of a husband and wife, type both names separated by the word "and". For example, Bryan and Melissa.
Email Address Family's email address.
Primary Phone Family's primary phone number.
Emergency Phone Family primary emergency phone number.
Emergency Ph. Desc. Description of the family's primary emergency phone number. For example, "My son, Michael's cell phone."
Envelope # Envelope number assigned to the family.

Description

Primary Address

Primary Address From the dropdown, select one of the following options to indicate which address serves as the family's primary address: Home Address, Mailing Address, Other Address.

Home and Mailing Tabs

Country The system defaults the country selection to the match the country of the logged-in user's organization. To change the country, select the desired name from the dropdown list.
Postal Code

If a postal code is required for the selected country where the home or mailing address is located, do one of the following:

  1. Type the postal code in the field. Based on the country selected, the field imposes the following character limits:
    • United States: five-character limit. A complete nine-digit postal code includes four additional digits (ZIP + 4).
    • Canada: six-character limit.
    • All other countries: variable length; 250-character limit.
  2. Click Postal code search icon.png (Postal Code search button) to find the postal code.

The Postal Code search button Postal code search icon.png is visible only if the form is in edit mode. Click edit details.png to put the form in edit mode.

After you enter the postal code, the system automatically populates the City and State/Region fields.

To remove the postal code along with the Plus 4, City, State/Region, and Country values for a selected address on the Home, Mailing, and Other sub-tabs, click X icon.png (Remove). The record must be in edit mode to remove these elements of an address.

Address Line 1 House or residence number and street name.
Address Line 2 Extra information to identify the address you entered for Address Line 1. For example, apartment number, floor number, suite number.
City City name (system-generated). The system uses the value entered in the Postal Code field to generate the city name. If more than one city is associated with the postal code entered, a dropdown list is displayed from which you can select the right city.
State/Region State or region name (system generated). The system uses the value entered in the Postal Code field to generate the name of the state or region.

Other Tab (if Other is selected for Primary Address)

Valid From Date range the family resides at the "other" address. Enter a date in mm/dd format. You can add these (snowbird) dates to your list of schedule exceptions in your ministry preferences record.
Country Select the county for the "other" address from the dropdown list.
Postal Code

If a postal code is required for the selected country where the "other" address is located, do one of the following:

Options to Publish Personal Information

Description

Send Mail

Checkbox control indicates whether or not the family wants to give the organization permission to send mail to their home:

  1. Select the checkbox to indicate that the family gives permission to have mail sent to their home.
  2. Leave the checkbox deselected to indicate that the family does not give permission to have mail sent to their home.
Publish Phone

Checkbox control indicates whether or not the family wants their phone number to appear in church publications (for example, Pictorial Directory and Parish Directory report):

  1. Select the checkbox to indicate that the family wants their phone number published.
  2. Deselect the checkbox box to indicate that the family does not want their phone number published.
Publish Email

Checkbox control indicates whether or not the family wants their email address to appear in church publications (for example, Pictorial Directory and Parish Directory report):

  1. Select the checkbox to indicate that the family wants their email address published.
  2. Deselect the checkbox box to indicate that the family does not want their email address published.
Publish Address

Checkbox control indicates whether or not the family wants their home address to appear in church publications (for example, Pictorial Directory and Parish Directory report):

  1. Select the checkbox to indicate that the family wants their home address published.
  2. Deselect the checkbox box to indicate that the family does not want their home address published.
Publish Photo for Pictorial Directory

Checkbox control indicates whether or not the family wants their family photo to appear in church publications (for example, Pictorial Directory and Parish Directory report):

  1. Select the checkbox to indicate that the family wants their photo published.
  2. Deselect the checkbox box to indicate that the family does not want their photo published.
